35mm slides, 100Mb Zip disk or CD-ROM - saved as JPEG (300dpi, no smaller than 105mm x 150mm) PDF, EPS or TIFF files, with a paper copy (no larger than A3 size) for reference.

Please include a listing of the work featured; details of agency, production co and post production; as well as the date it was first shown (or date completed, if unaired).
